Transitions strengthens patient
care through better
coordination of medical services.
Too often, a patient is admitted into the hospital for pain or symptom
control and then discharged...only to be re-admitted when the pain
or symptoms re-appear. The cycle of admission and discharge repeats.
Transitions provides critical case management services to a
physician or other health care provider who is following a
discharged patient.
Emotional and non-medical support help
patients and their families The
program’s services vary depending on whether the patient lives
in
his or her own home or in a nursing home.
If a patient lives at
home, then hospice sends a home
service volunteer who offers:
> Friendship and visitation
> Help with writing and reading correspondence
> Running errands
> Food preparation
> Respite breaks for the family
If the patient lives in a nursing home,
then a nursing home volunteer offers:
> Friendship and visitation
> Help with writing and reading correspondence
