A plan of care

We begin with a plan of care involving the nurse or therapist, and you and your caregiver. RNs and LVNs are available to help you and your family with education, assessment and observation of wounds, respiratory status, cardiac status and other body systems. They also administer IVs, wound care, colostomy care, catherter care and cathether changes.

Home Health Aides may assist you with personal care needs, exercises taught by a therapist or a nurse, meal preparation and other homemaking needs.

A Licensed Physical Therapist will tailor a home program to your individual needs. The LPT will address safety concerns, as well as lower extremity and whole body strengthening.

A Licensed Occupational Therapist will assess your needs and then design a plan for treatment that includes assessment for safety, focusing on upper body strength. The OT also can assess, recommend and fit you with adaptive devices as needed.

A Licensed Speech Therapist can assess and treat speech difficulties, swallowing problems and disrupted thought processes.

A Medical Social Worker is available to help with problems related to care at home, depression, long-term planning and coordination of essential community services.

A Homemaker Service is also available that provides light housekeeping, errands, meal preparation and companionship.

Also available is Lifeline, a personal response and support system. With Lifeline, help is just the press of a button away. You may request them yourself, or your physician may order them.

Who pays for these services?

All services are available through private pay. Sometimes, when a physician orders them, skilled services may be covered by private insurance. Aide and homemaker services are available through private pay.
