but HomMed, an in-home system, offers clients monitoring on those
days a nurse isn't scheduled to make an in-person call. |

About in-home monitoring |
|
The HomMed Health
Monitoring System collects your clinical data such as heart rate,
blood pressure, oxygen saturation levels, weight and temperature all
from your home.
Home Care nurses continue to make home visits, but the HomMed system
offers you daily monitoring when home visits are not scheduled.
The HomMed system can detect some problems before they become more
serious.

How in-home monitoring works for
you |
|
The system uses an
advanced voice-instruction module that instructs you step by step,
what to do.
Your data collected is transmitted over telephone lines to the home
care agency. If a daily monitoring report is not received on
schedule, the agency contacts your home to find out if assistance is
needed.

Who can benefit
from in-home monitoring? |
|
Typical patients are
those with chronic conditions such as hypertension, congestive heart
failure and chronic obstructive pulmonary disease (COPD), as well as
certain post-operative cardiac patients.

Who pays? |
|
Medicare-eligible
patients who are referred to HCM Home Care Services and who meet the
selection criteria will be monitored at no cost. Patients not
covered by Medicare have the option of being monitored for a nominal
fee.
