patients receive special end-of-life care in the comfort of familiar
surroundings, at home or in their nursing home. |

What is hospice? |
|
When patients and
families choose hospice, they receive care unlike any other. Hospice
goes to the patient, treats pain and symptoms with aggressive
medical care and eases emotional and spiritual suffering.
Hospice care emphasizes living as fully as possible, sharing special
family experiences and returning control to the patient.
An interdisciplinary hospice team provides care that includes coordination
among the primary physician, the hospice medical director,
nurses, social workers, chaplains, home health aides, counselors and
volunteers.

qualifies for hospice? |

Inquiries and referrals may be made by
anyone.
Admission criteria include: |

Life-limiting diagnosis with a prognosis measured in months if
it follows its expected course |

Consent of the primary care physician |

A
caregiver willing to provide patient care |
| > |
Residency in the hospice service area |
|
|
Hospice services are
available to all persons, regardless of financial status. Services
are based on need rather than ability to pay. Hospice care may be
reimbursed by Medicare, Medicaid or private insurance.
